Deadline for sign-up, forms & payment is at Tuesday meeting, 9/8. No late sign-ups. Please deliver all documents and payment to me at the Scout Hut during 9/8 meeting, I will be there till 9:30 PM no later.

Download attached four documents. They will provide all campout details, instructions and forms regarding:

Sign-up sheet
Fee for rental of raft or Ducky
Ducky qualifications
Waiver form for outfitter (Nantahala Outdoor Center) (Parent & Scout must sign Scouts Waiver Form)

Dads are strongly encouraged to join us as we will need additional drivers and supervision on the river. If Dads don't want to float, we still need you. This event is our most popular of the year and participation requires
more adults than we have registered Scouters available to attend or to handle. We greatly appreciate your understanding, consideration and sign-up.

Scouts & Scouters attending: BSA Medical Forms Must Be Current (Parts A, B, & C) to participate. Please ensure medical forms are current (less than one year old). If content of Parts A & B are still correct, change date and initial.
Part C must be filled out annually, signed and dated by a medical practitioner. Check your status on front page of Troop web site ( https://www.troop232atl.com ) or email Assistant Scoutmaster, Dodge Sumlin at: dodge.sumlin@gmail.com

BSA medical forms are attached along with instructions, the forms are also available at: http://www.scouting.org/filestore/HealthSafety/pdf/whole.pdf

Dads attending: We must also collect just Parts A & B of Medical Forms for Dads in event of an emergency (these two pages do not require a physical).

Make a copy of medical forms for home records / future use. Mail original forms with copy of front & back of insurance card to:

Dr. Dede Casal
215 Powers Cove
Atlanta, GA 30327

All medical forms are kept in our traveling medical binder. They are confidential and as such, are kept locked in a secure cabinet at the Scout Hut when not traveling on a campout.
